Refer to the New Dental Choice membership card

New Dental Choice members should be able to provide you a membership ID number, either from their membership card or the email they received when they joined. The card itself doesn’t guarantee active membership because sometimes a member may have an old card. Your best bet is to check online or call us at 888-632-8585 to confirm a patient is a New Dental Choice member.
